Subject: * init.cc (in_dllentry): Delete. (dll_entry): Remove assignment to
 deleted variable. * winsup.h (in_dllentry): Delete declaration. *
 exceptions.cc (inside_kernel): Use another method to see if we are in
 dll_entry phase.
