Use CharNextExA to recognize multibyte characters in input. Workaround
problem with UTF-8 and MultiByteToWideChar. Simplify the loop for
printing "normal" characters.

(get_cp): Call codepage_init() if active_codepage is uninitialized.
Just return active_codepage.
