The sample aspect ratio is a per-frame property, so it makes sense to
define it in AVFrame rather than in the codec/stream context.
Simplify application-level sample aspect ratio information extraction,
and allow further simplifications.

a change of behaviour that was not discussed.
The reason for the revert is that FFmpeg cares about bugreports.

This way, all messages can be easily disabled with -loglevel.
Behavior change -- configuration is printed with AV_LOG_VERBOSE, so it's
not shown by default.
