Code Cleanup:
* made bmesh_structure.h function names more consistant.
* remove unused code in bmesh_structure.c
* removed 'Edge Flip' operator (missing from bmesh but looked into trunk feature and dont think its worth keeping).
* tagged some BMESH_TODO's
