It is supposed to be two spaces before comment stating which if
else/endif statements corresponds to. Was mainly violated in the
header guards.

MX (Multiple conteXt) support was dropped from the GLEW 2.0 library to make core profile support cleaner.
Our WITH_GLEW_MX build option was OFF by default already; this commit removes the inactive code paths.
I'm working on a plan for multiple GPUs, contexts, resource sharing, etc. This commit gives us a cleaner starting point for that upcoming work.
Tested on Mac, will test on Linux & Windows immediately after pushing.

This commit contains all the tweaks which were missing in initial patch
re-integration from the standalone Cycles repository.
This commit also contains an utility cmake macro to help linking targets
with different libraries for release/debug builds, the name currently is
target_link_libraries_decoupled
it gets a target and list of libraries and makes sure debug builds are
using libraries with "_d" suffix.
After all this changes it'll hopefully be easier to interchange patches
between blender and standalone repositories, because they're now quite
identical.

This was already mixed a bit, but the dot belongs there.

Separates graphics context creation from window code in Ghost so that they can vary separately.

* 32 bit GCC builds now have the SSE BVH optimizations turned off, but still
compile with SSE flags for better performance.
* White color when rendering on Windows seems to have been unrelated to SSE,
rather it was a graphics driver not supporting half float textures, added a
check for that now.

modifications and build instructions will follow later.
Cycles uses code from some great open source projects, many thanks them:
* BVH building and traversal code from NVidia's "Understanding the Efficiency of Ray Traversal on GPUs":

* Open Shading Language for a large part of the shading system:

* Blender for procedural textures and a few other nodes.
* Approximate Catmull Clark subdivision from NVidia Mesh tools:
