Move \ingroup onto same line to be more compact and
make it clear the file is in the group.

BF-admins agree to remove header information that isn't useful,
to reduce noise.

Developers should add non license comments as separate comment blocks.
No need for separator text.

This is often invalid, outdated or misleading
especially when splitting files.
It's more useful to git-blame to find out who has developed the code.

New code is over three times quicker than old one here (e.g. Suzanne
subdiv level 4, 250k tris, threaded part is now 1.4ms instead of 4.5ms
with OMP).

This helps avoiding threading overhead when having lots of system threads..

Those pointers are never to be aliased, so let's be explicit about this and hope
compiler does save some CPU ticks.

Now all the fine-tuning is happening using parallel range settings structure,
which avoid passing long lists of arguments, allows extend fine-tuning further,
avoid having lots of various functions which basically does the same thing.

Wrap all arguments into TLS type of argument. Avoids some branching and also
makes it easier to extend things in the future.

Performances gain is rather low this time, about 5% on average (in whole
affected functions).

yet another bug introduced by recent shadowing changes -- q and r CCG arrays
were overwritten by the temporary evaluation because the code was changed to
use global pointers instead of the local ones.
Naming of the variables could be changed to be a bit more accurate.

This is a preparation commit for having OpenSubdiv integrated into Blender
and new layout is the following:
- CCGSubSurf.c contains implementation of common functions used by both
legacy subdivisions code and by the new code in the future.
- CCGSubSurf_inline.h contains internal functions which are to be inlined
due to the performance reasons. Those functions are only ment to be used
bu CCGSubSurf* files.
- CCGSubSurf_intern.h contains declarations of private functions and data
structures used by CCGSubSurf module.
- CCGSubSurf_legacy.c contains legacy implementation of subdivision algorithm.
- CCHSubSurf_util.c contains utility functions which are not directly related
on the subdivision code (i.e. debug functions, hash implementation etc).
There should be no functional changes so far.
