BF-admins agree to remove header information that isn't useful,
to reduce noise.

Developers should add non license comments as separate comment blocks.
No need for separator text.

This is often invalid, outdated or misleading
especially when splitting files.
It's more useful to git-blame to find out who has developed the code.

Was using an edge hash for triangle -> edge lookups,
updating triangle indices for each edge-rotation.
Replace this with half-edge which can rotate edges much more simply,

`CustomData_bmesh_interp_n` was expecting the 'dest' arg not to have its offset applied.
This was a bit confusing since the source args have it applied,
and in some cases we only have the destination with the offset.

Turned out there was still quite a few cases were indices were set dirty,
but elem_index_dirty was not tagged accordingly (mostly for BM_LOOP,
but a few others as well). So probably this crash was not the only one
hidden here.
Hopefully all possible cases were catched this time!
