Merge branch 'ab/fewer-the-index-macros'
Progress on removing 'the_index' convenience wrappers.

builtin/{grep,log}.: don't define "USE_THE_INDEX_COMPATIBILITY_MACROS"
cache.h: remove unused "the_index" compat macros
