Merge branch 'tb/refs-exclusion-and-packed-refs'

Enumerating refs in the packed-refs file, while excluding refs that
match certain patterns, has been optimized.

  ref-filter.h: provide `REF_FILTER_INIT`
  refs.c: rename `ref_filter`
